Community concern or referral can connect to specialist evaluation. Where a documented SCF support pathway applies, the Foundation's contribution may accompany treatment navigation and continuity.

Establishes whether there was a Foundation referral, eligible support, navigation or follow-up relationship.
Uses only reconciled aggregate evidence. An unmatched Santosh treatment record is never presented as an SCF-supported case.
The purpose is not simply to record a camp, referral or supported intervention. The aim is to help the person remain connected with the appropriate healthcare pathway wherever practical.
